Introduction

The Regional Director of Care Coordination leads the implementation and management of a care coordination model with effective, sustainable processes and structure that successfully supports performance with accountable care organizations, CMS programs, payer specific quality programs and other quality, payer, and government initiatives as directed.

The Regional Director of Care Coordination facilitates growth and development of the Care Coordination program consistent with PSG’s philosophy and responsible for leading a team of both clinical and nonclinical staff across multiple divisions in the execution of exemplary care coordination in the ambulatory environment.

It's preferred for candidates to be based in Florida or willing to relocate to support our divisions. While this role is considered Work from Home, travel across Florida, Georgia, South Carolina and other key markets could be required.

Job Summary and Qualifications

  • Provides leadership in the development, implementation, execution, and evaluation of an effective regional care coordination program that supports the delivery of quality healthcare in the most appropriate and effective manner. Program must meet the obligations of contractual and regulatory requirements and the short-term and long-term goals of PSG

  • Provides leadership, guidance and structure to care coordination team members that include the following: RN Care Managers, LCSW Care Managers, Care Coordinators

  • Maintains comprehensive knowledge of payer and regulatory requirements with ability to work effectively under pressure and to adapt to changing priorities. Keeps abreast of industry trends and guides team to adapt to meet evolving needs of payers, government programs and the healthcare industry

  • Responsible for the planning, priority setting, coordination, implementation, evaluation and reporting of programs related to patient care coordination

  • Develops systems to ensure effective communication and coordination between the care coordination team, division and group leadership, PSG operations, and Provider Contracting and Alignment

  • Effective influencing, negotiation, relationship-building and communication skills are essential with the ability to effectively interact and establish immediate credibility with all levels of the region’s management teams, physicians, staff and payer representatives

  • Collaborates with interdisciplinary teams and leaders to achieve the organization’s coordination of care goals, quality goals, and financial performance goals

  • Develops regional goals and objectives, and establishes and implements policies and procedures for regional care coordination operations

  • Assists in directing the preparation and maintenance of care coordination reports and prepares periodic reports for senior management

  • Ability to think strategically and anticipate future changes with a performance excellence, outcomes-focused mindset.

  • Provides leadership to drive care coordination for clinical improvement and service enhancement

  • Monitors and communicates with Operations regarding care coordination trends. Assists in preparations of metric reports and development of meaningful improvement plans

  • Facilitates effective communication with Physician Services Group executive leadership regarding key care coordination activities and initiatives

  • Maintains the strictest confidentiality in the areas of patient, employee and physician relations.

EDUCATION:

  • Bachelor’s degree required. Master’s degree preferred.

EXPERIENCE:

  • Minimum 3 years of related clinical and management experience involving care coordination, ambulatory care, quality improvement, leadership, and healthcare quality practices, as well as associated regulatory rules and laws. At least one year must have been in capacity as direct supervisor of other professional level employees.

CERTIFICATION/LICENSE:

  • Current licensure as a Registered Nurse in state of residence
