Key Responsibilities

  • Conduct initial and follow-up assessments to develop personalized treatment plans.

  • Provide therapeutic exercises and treatments to improve patients’ physical abilities.

  • Educate patients and their families on exercises, techniques, and lifestyle changes to enhance their progress.

  • Collaborate with a multidisciplinary team to ensure the best patient outcomes.

  • Document patient progress and communicate effectively with team members and family caregivers.

What We’re Looking For

  • Active PT License in Pennsylvania .

  • Experience in home health preferred, but not required – we’re happy to train passionate professionals.

  • Strong interpersonal skills and compassion for patients in various stages of mobility and health.

  • Ability to adapt therapy techniques for the home environment.
