Key Responsibilities
Conduct initial and follow-up assessments to develop personalized treatment plans.
Provide therapeutic exercises and treatments to improve patients’ physical abilities.
Educate patients and their families on exercises, techniques, and lifestyle changes to enhance their progress.
Collaborate with a multidisciplinary team to ensure the best patient outcomes.
Document patient progress and communicate effectively with team members and family caregivers.
What We’re Looking For
Active PT License in Pennsylvania .
Experience in home health preferred, but not required – we’re happy to train passionate professionals.
Strong interpersonal skills and compassion for patients in various stages of mobility and health.
Ability to adapt therapy techniques for the home environment.
