POSITION SUMMARY

The Behavioral Health Case Manager is responsible for managing an assigned caseload of clients having long-term mental illness and for assessing client needs, developing, implementing and reviewing service plans, and working with other internal and external resources in meeting/achieving client service needs.

Qualifications

ESSENTIAL SKILLS AND EXPERIENCE:

  • Bachelor’s Degree in Social Service field is preferred or a minimum of five (5) years of behavioral health experience and one (1) year of direct experience may replace Bachelor’s Degree

  • High school diploma or GED from an accredited institution is required

  • Minimum of one (1) year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients; preferred

  • Excellent customer service skills

  • Excellent computer skills

  • Knowledge of Arizona Department of Health Services and RBHA requirements

  • Thorough knowledge of current versions of DSM and ICD

  • Ability to complete a Behavioral Health Assessment with appropriate clinical oversight

  • Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices

  • Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross

  • Bilingual English/Spanish strongly preferred

  • Valid Arizona driver’s license

  • Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety

    POSITION RESPONSIBILITIES/ESSENTIAL:

  • Participate as directed in the screening of new service requests within the center ensuring that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and procedures

  • Participate in regular interdisciplinary staff meetings

  • Accept newly assigned cases in which the diagnostic intake has been completed by the Intake Specialist. Completes case management assessment on all clients entering caseload

  • Develop appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clients

  • Provide direct client services to individuals and/or family members. Services provided may include case management, skills training, group counseling, group skills training, health promotion, crisis intervention, transportation, supportive employment services, counseling with appropriate clinical oversight and others depending on client need. Services may be provided in office, home, school or community settings

  • Provide on-going supportive and/or case management functions in accordance with the problems, needs and the strategies identified within the service plan in order to assist the client to achieve goals and objectives

  • Provide face to face reviews with the client and/or relevant behavioral health staff on a regular basis. Ensures progress made is reported so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency reviews will be determined by relevant requirements

  • Assist supervisory staff with tracking of clients, including the generation of reports that will allow follow-through on quality of care

  • Document all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with internal and regulatory standards requirements

  • Maintain close communication with the consulting psychiatrist and the Behavioral Health team for input regarding medication compliance, side effects of medication, and medication changes and alerts behavioral health team of any changes in client adjustment which might suggest decompensation and a need for more aggressive intervention
