CASE MANAGER OSN HOME SERVICES

POSITION PURPOSE:

Provide quality services in a community environment with the skill sets to manage complex situations related to individuals who have experienced homelessness, potential substance use/behavioral/emotional problems.

WHAT YOU WILL DO:

Client Assessment : Conduct initial assessments of individuals experiencing homelessness to identify their needs, barriers, and strengths.

Case Planning : Develop individualized service plans in collaboration with clients, setting goals and outlining steps to achieve housing stability and self-sufficiency.

Resource Coordination : Connect clients with community resources such as shelters, food assistance programs, healthcare services, mental health counseling, substance abuse treatment, and job training programs.

Advocacy : Advocate on behalf of clients to access housing, public benefits, legal assistance, and other necessary services.

Crisis Intervention : Provide immediate assistance and support during crises such as housing emergencies, mental health crises, or substance abuse relapses.

Regular Follow-Up : Maintain regular contact with clients to monitor progress, address challenges, and adjust service plans as needed.

Documentation and Reporting : Keep accurate and up-to-date records of client interactions, service provision, and outcomes. Prepare reports for funders, supervisors, and stakeholders as required.

Collaboration : Work closely with other service providers, government agencies, nonprofit organizations, and community partners to coordinate services and support for persons served.

Cultural Competence : Demonstrate sensitivity and cultural competence in working with diverse populations, including individuals from different ethnic, racial, cultural, and linguistic backgrounds.

Professional Development : Stay informed about best practices, trends, and developments in homeless services, attending trainings and workshops as necessary to enhance skills and knowledge.

Ethical Standards : Adhere to professional ethics and standards of conduct, maintaining confidentiality and respecting the rights and dignity of clients at all times.

Self-Care : Practice self-care strategies to prevent burnout and maintain personal well-being while working in a demanding and emotionally challenging field.

MINIIMUM QUALIFICATIONS:

  • Masters Degree in Social Services with 1 year of related social service experience OR

  • Bachelors Degree in Social Services with 3 years of related social service experience OR

  • Associate's Degree in Social Services with 4 years of related social service experience OR

  • High-school diploma/GED and 8 years of related social service experience

  • Act 33/34/73 Clearances

  • Valid Driver License

  • Ability to drive an organization vehicle

  • Pre-employment drug screen

POSITION HIGHLIGHTS/BENEFTIS:

Work Schedule: This is a Non-Exempt (hourly) position. Monday – Friday, 8am-4pm, with flexibility to meet consumer and/or program needs. Some non-traditional hours (evenings, weekends, and holidays) may be required.
