CN Central Nassau Guidance & Counseling Services Care Navigator in Plainview, New York

The Care Navigator conducts clerical based office work, assists in monitoring clients, and alerts members of the Health Home Care Management (HHCM) program when follow-up is required. Provides electronic and/or telephonic support to staff to facilitate improving client care. In addition to office tasks, Care Navigators maintain the upkeep of agency vehicles and interdepartmental mail at other offices.

EDUCATION REQUIRED:

High School Diploma/GED

Bachelors preferred

EXPERIENCE REQUIRED:

Experience in office-based setting.

Knowledge of human service delivery.

Valid New York State Driver's License

DUTIES AND RESPONSIBILITIES:

Responsible for office tasks such as filing, uploading documents, creating, and closing charts via electronic health record and physical binder. Responsible for assisting the care coordinators and other team members. Responsible for answering the phone and alerting or bringing important follow-up items to the care management team's attention. Outreach via phone to clients following hospitalizations as assigned. Ensure that the corresponding team members receive client alerts (e.g. ER Visits, admitted, discharge, gaps in care, etc.) in a timely manner. Conduct quality service phone calls to clients as assigned. Provides general clerical support (e.g., faxes documents, arranges team meetings, take meeting minutes, etc.) Responsible for agency vehicles by keeping up with inspections, service calls, etc. Participates in committees as directed. Attends required in-service programs. Participates in supervision, training and education as needed. Assisting management team as needed Compete QI training during onboarding and participate in QI refresher training as need-ed Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
