Crossroads Hospice & Palliative Care RN Case Manager - No Weekends in Plymouth Meeting, Pennsylvania

As a Registered Nurse Case Manager, you will provide direct care to patients in their homes or care facilities. Your role goes beyond offering medical assistance; it involves attentively listening to patients' needs and respecting their wishes. Building trusting relationships with patients and their families is a cornerstone of this position, allowing you to serve as the primary communication link within our interdisciplinary team of holistic care providers.

Registered Nurse Case Manager Responsibilities:

  • Provide direct patient care, including assessing and monitoring patients' conditions, administering medications, and performing treatments

  • Collaborate with healthcare team members to develop and implement individualized care plans for patients

  • Coordinate and facilitate communication between patients, families, and healthcare providers

  • Conduct ongoing assessments to evaluate the effectiveness of the care plan and make necessary adjustments

  • Educate patients and their families on disease management, treatment options, and self-care techniques

  • Utilize case management principles to ensure continuity of care and appropriate resource utilization

Registered Nurse Case Manager Benefits & Schedule:

  • Monday to Friday, 8:30 AM to 5:00 PMĀ 

  • No Mandatory on-call required
