The LVN Case Manager will perform clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating). This includes case management, coordination of care and medical management consulting within an interdisciplinary team.
Position in this function provides health education, coaching and treatment decision support for members.

Required Qualifications:
Current unrestricted NY LVN license
2+ years of experience in a physician’s office, clinical or hospital setting
Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
Proficient computer skills to work efficiently with electronic medical records
Ability to travel within a commutable distance of Poughkeepsie for internal and external meetings
Preferred Qualifications:
Experience related to patient education and/or motivational interviewing skills and self-management goal setting
Case management experience
Comfortable speaking and presenting to hospital partners and executive leadership
New York Residents Only: The hourly range for this role is $19.47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
