Description of Job

The Case Manager position is a full-time (40 hours per week) administrative professional position with flexible hours which may include nights and weekends. The Case Manager provides non-clinical case management services that connect students with appropriate resources. Duties include responding to referrals; developing and coordinating interventions; tracking student completion of recommendations; and following up to ensure student success. The Case Manager also oversees the Peer Mentoring Program on campus and promotes peer mentoring across the university. The position supports students with diverse identities including first generation college students and Hispanic students in a holistic fashion that contributes to their overall wellness and success.

This position is funded through September 30th, 2025 by a Title V grant. Funding may be extended beyond that date by No Cost Extensions from the Department of Education.

Primary Duties

Pack CARES & Non-Clinical Case Management

  • Provide culturally sensitive case management services to students of concern regarding matters impacting their health and academic success.

  • Conduct comprehensive assessments of student support needs.

  • Develop and implement individualized service plans tailored to each student's circumstances, including short-term interventions and long-term support strategies.

  • Provide ongoing monitoring and follow-up support to ensure students’ needs are met and goals are achieved.

  • Attend Pack CARES Team meetings and report progress related to student goals and utilization of resources.

  • Maintain repository of university and community resources and update procedural manuals and guides as needed.

  • Provide Pack CARES and Maxient onboarding training when assigned.

  • Contribute to efforts involving assessment and reporting of key metrics related to case management.

Peer Mentoring Program

  • Train peer mentors on resource navigation and methods to enhance student success.

  • Plan and implement activities for the peer mentor program.

  • Collaborate with other university departments to extend peer mentoring opportunities for undergraduate and graduate students.

  • Collaborate with Mentor Collective to recruit mentees.

  • Distribute marketing materials to recruit new mentees and mentors.

  • Supervise paid peer mentor positions.

Ancillary Duties

  • Build relationships with community resources to promote continuity of care and promote student success. Participate in relevant professional development.

  • Serve on university and Division of Student Affairs committees as appropriate.

  • Other duties as assigned.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications

  • Bachelor’s Degree in social work, nursing, psychology, education, health studies or related field

  • 2 or more years of professional work experience in higher education or field related to case management

  • Experience documenting success plans, interventions, goals, and outcomes in a secure database

  • Experience in a position or activity that required communication skills that contributed to effective collaboration with students, faculty, and staff

  • Experience working with diverse student populations

  • Experience with, or commitment to, working in and with a culturally diverse community

  • Experience with, or requisite training, to provide non-clinical advising to students related to personal and educational matters

  • Experience in a position or activity that required an awareness of referral sources for students with health, mental health or behavioral concerns both on and off campus

Preferred Qualifications

  • Master’s degree in social work, nursing, psychology, health studies or related fields

  • Knowledge of learning methods of college populations.

  • Ability to utilize current research and guidelines to develop meaningful educational materials for the college age population

  • Excellent written and verbal communication skills

  • Ability to manage referrals and track students’ successful completion of health, mental health and behavioral challenge success plans and report to appropriate parties including Counseling, Pack CARES Team and Student Conduct

  • Experience leading campus/community groups or coalitions

  • Experience working with peer educators

  • Experience in data collection, analysis, and program evaluation

  • Experience with student information software such as Starfish or Maxient
