Job Title: RN Care Delivery Manager OR Clinical Review Nurse
Education: Degree- RN/ BSN or 5 years' experience!
NLN-accredited nursing school (RN, BSN required or at least five (5+)years of experience in health care setting).
A Bachelor degree is highly desired, not required
Experience:
Minimum of one (1+) year of experience in
home care as a community health nurse in a Certified Home Health Agency, Long
Term Home Health Care Program, and/or Licensed Agency, or with the frail, elderly, or geriatric population
Experience in a PACE Program or MLTC preferred.
Computer/laptop proficiency required.
Managed long-term care insurance experience beneficial.
License:
Current active and unrestricted license and registration in New York State as a
Registered Nurse, required.
Job Responsibilities:
Will help Community Health Nurses with more complex cases and assist in completing competencies.
Will supervise the Community Health Nurses and help with case management, Will hold their own case load.
Responsible for supporting the nursing standards of excellence with advanced professional education to deliver high-quality care.
Serve the organization as a member of the management team, working alongside all departments and employees to support and oversee day-to-day operations.
Provide guidance and deliver best practices in achieving the organization's objectives, goals, and mission.
Cultivate an environment dedicated to improving experience, enhancing satisfaction, and retention.
Work Site
Queens or Manhattan, New York.
