Insight Global Blended Case Management Supervisor in Reading, Pennsylvania

Job Description

Insight Global is working with a recovery-oriented behavioral health, physical health, and supportive services organization providing a holistic approach to a culturally and economically diverse population for the purpose of achieving a healthy and productive lifestyle.

They are looking to hire for a full-time, permanent Behavioral Health Counselor.

As a Blended Case Management Supervisor, you will supervise blended case managers and work closely with teams to ensure appropriate use of case management services.

Day-to-day

  • Oversee Blended Case Management programming as well as future social programs to be developed.

  • Provide consultation to teams on addressing Social Determinants of Health.

  • Actively participate in high engagement team huddles

  • Monitor and assess services received by clients. - This will involve frequent and consistent contact with the clients to determine progress level and working to modify areas of the plan that may not be as effective as needed.

  • Hire, supervise, and evaluate program staff in the above areas of responsibility.

  • Perform routine review of charts and service plans to ensure appropriate level of effectiveness.

Skills and Requirements

  • A bachelors degree in human services or related field.

  • One (1) year of mental health direct care experience.

  • Working knowledge of Blended Case Management and Team Based Care.

  • Strong management, program development and oversight, and community networking skills

  • Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable. - Have a masters degree in a human services or related field.

  • Have Knowledge of or experience with trauma-informed approaches preferred. null
