Berks Counseling Center Health Coordinator in Reading, Pennsylvania

As a *Health Coordinator you will *be an active member of the integrated care team and important role in bridging the medical and clinical departments for a more efficient client experience.

You will...

  • Support the client in completing an integrated clinical assessment.
  • Assist client in acquiring a primary care physician.
  • Communicate with pharmacies and community providers to ensure collaboration and coordination of care.
  • Monitor patients' health needs by completing and updating health screenings: taking vital signs; height and weight; reviewing and/or recording medical history.
  • Facilitate referrals to internal and external health providers.
  • Work with patients to ensure their engagement in services with a primary care physician.
  • Collect urine drug screens, log the specimens, and prepare paperwork for clinical staff to review for intake.
  • Completes medical records by recording patient examination, treatment, and test results. 
  • Assists with obtaining medical records from previous providers to ensure continuity of care.

To qualify for the position, you will need...

  • An associate degree in public health, exercise physiology, nursing, dietary or a related field.
  • Minimum of two (2) years in health promotion, wellness, or health education.
  • A valid PA Driver's License and the ability to drive, travel, and provide community-oriented services.
  • Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.

Preferred candidates will ...

  • Have a bachelor's degree in public health, exercise physiology, nursing, dietary or a related field. 
  • Have at least 1 year training or experience relevant to working in a medical office with mental health or substance use treatment preferred.
  • Have Certifications in health coaching from an accredited professional or institute.
  • Have Basic Life Support certified.
  • Be Bilingual: English/Spanish.
  • Have Knowledge of or experience with trauma-informed approaches preferred.

*Physical Requirements: *

  • SEEING: Must be able to see well enough to read reports and use computer.
  • HEARING: Must be able to hear well enough to communicate to clients, vendors, and coworkers.
  • MOBILITY: Must have mobility to move about the agency. Must have mobility to move about the community. LIFTING/PULLING/PUSHING: Must be able to lift/pull/push up to 15 pounds.
  • OTHER: Must be able to write, type and use technology.

  • Receipt of results of a satisfactory drug and alcohol screening test. 
  • Receipt of the originals of the following background checks: PA tate Police Criminal History, PA State Child Abuse, and DPW FBI Criminal History. These clearances must be valid within the 60 months prior to employment. A conviction is not a bar to employment; however, a review will be conducted to make a determination of employment should a conviction be present.
  • Receipt of original documentation and verification of relevant degrees, diplomas, certificates, or licensure.
  • Confirmation that you are not on the OIG Federal Healthcare Program Exclusion from Participation list. Should you appear on this list, Berks Counseling Center is prohibited from employing you.

Salary range: Associates degree: $37,300 to $39,500.  Bachelor's degree: $38,800 to $41,100
