    JOB SUMMARY

    The Director of Social Services (DSS) leads the social service unit that develops and executes service and care plans for residents to provide a supportive and empowering environment that fosters stabilization, independence and self-reliance, and skills development, and enhance the health and general well-being of residents; and ensures services are appropriate and culturally sensitive. The DSS provides operation and administrative support to ensure program and contractual compliance and ensures the accurate and punctual submission of required documentation and reporting to CHI, city, state, and federal agencies. The Director builds and sustains a strong service network with external providers and community-based organizations to support staff and assist residents towards stability and independence.

    Pay: $78,000 salary.

    FLSA: Exempt. Not eligible for overtime.

    Schedule: 11am-7pm Tuesday- Saturday (Off Sunday & Monday)

    JOB-RELATED DUTIES – ESSENTIAL FUNCTIONS

    These duties are essential and specific to the successful implementation of this position.

    SERVICE AND CARE COORDINATION

  • Ensure residents receive quality and appropriate services per organizational principles and program requirements.

  • Develop and maintain a resource network, inclusive of formal and informal partnerships and service agreements, of external providers and organizations that offer support, advocacy and direct services for immigration and citizenship issues, health and mental health issues, employment, education, childcare and youth service connections, trauma recovery and counseling, cultural adaptation, and English language acquisition, life skills and independent living skills. Ensure program team leverages network partnerships in service planning and execution.

  • Ensure Social Worker and Case Management teams develop service and care plans that are aligned with biopsychosocial assessments. Ensure assessment and plans provide a culturally sensitive and comprehensive analysis and identify appropriate and adequate services and resources.

  • Ensure recreation workshops are engaging, promote education, employment, self-care and healthy habits, cultural adaptation, English language proficiency, communication skills, and independent living skills.

  • Identify and codify best practices; coach program team to implement best practices.

  • Review discharge plans to ensure continued enrollment and participation in public benefits programs and service linkages to community-based organizations are in place.

    PROGRAM MANAGEMENT

  • Ensure program deliverables are met. Develop enhancement strategies for underperforming areas. Ensure CHI program KPIs are aligned with program metrics and deliverables.

  • Ensure proper and timely follow-up to Priority 1 and 2 incidents with clients, and reporting to DHS and CHI.

  • Ensure program team adheres to CHI code of conduct and abide by contractual, city, state, and federal rules and regulations.

    SUPERVISION

  • Directly supervisor Case Management Supervisor, Social Work Supervisor, and Recreation team.

  • Troubleshoot difficult resident cases; intervene and directly assist in special needs cases.

  • Conduct 1 to 1 meetings with members of your team and update the performance goals, accomplishments, notes, and feedback in Paycom on a regular basis (weekly/bi-weekly).

Qualifications

What do I need to apply?

Education and Experience: Master's degree in Social Work or related field required. LMSW or LCSW preferred. Eight years of experience plus three years of supervisory and managerial experience in social work, case management, counseling, or direct advocacy for clients in social service settings. Experience with immigrants and immigration issues prioritized.

A bachelor’s degree in social services and ten years of experience in social services with 5+ in a supervisor setting can be used in lieu of the education requirement.

  • Communication:Professional and impeccable verbal and written communication skills. Proficiency in English required, multi-lingual and Spanish proficiency prioritized.

  • Computer skills:Professional and expert competency in MS Word, Excel, Outlook and the internet. Experience with CARES, HOME, CAPS, and AWARDS prioritized.

  • Math Skills:Ability to formulate financial budgets and coach budget management skills.

  • Physical Performance:Ability to make unit visits (walk distances, climb stairs, etc.)

  • Reasoning Ability:Ability to prioritize, make appropriate decisions and judgment calls.

  • Other Skills

  • Extensive experience conducting and reviewing biopsychosocial and needs assessment surveys, service and care plan design and execution, and trauma and recovery counseling.

  • Extensive experience working with homeless individuals and people living with health and/or mental health issues and substance use. Ability to engage residents.

  • Extensive experience in building and sustaining partnerships and collaborations with external service providers and community-based organizations.

  • Extensive experience in compliance management and contract management.
