Delphi Rise Health Home Care Manager in Rochester, New York

Pay Range: $21.00-$23.00 Per Hour (depending on education, experience and qualifications) Pay Differential: Additional $2.00 per hour for bilingual proficiency in Spanish.
Summary: Health Homes Care Managers coordinate various aspects of client care with outside providers, including chemical dependency, mental health and medical services, accessing benefits (Medicaid, Medicare, food stamps, etc.), locating safe housing and assessing and supporting the social determinants of health.
Job Responsibilities: Assist clients with issues related to benefits, housing, childcare, healthcare, and other recovery supports that might serve to support their success. Coordinate client care with other health care providers (primary care doctors, treatment counselors, etc.). Complete assessments and supporting documentation in a timely fashion by utilizing the Netsmart Care Management system. Keep up-to-date accurate electronic case notes in accordance with rigid DOH and HHUNY standards. Make appropriate referrals and provide follow-up. Perform other tasks as assigned.
QUALIFICATIONS: The minimum requirements for this position: Education: A bachelors degree in a behavioral health, mental health, education, or social work-related field; OR a Credentialed Alcoholism and Substance Abuse Counselor (CASAC) certification. Experience: With a bachelors degree, five years of experience supporting and/or providing direct services to individuals impacted by substance use disorders, behavioral, and/or developmental health diagnoses is strongly preferred. A masters degree with one year of experience is acceptable. Cultural humility in all interactions, and a willingness to uphold our values of diversity, equity and inclusion. Excellent computer skills and ability to utilize a complex, web-based care management system. Vast knowledge of resources in the community that meet a wide variety of client needs. Comfortable working independently and yet highly collaborative in approach.
