LPN Family Care Coordinator - Assisted Living Memory Care
Full Time
Salary Range: $27.00 - $33.00 per hour

RPH is seeking an LPN to take on a leadership role as the Family Care Coordinator at our Memory Care Residences located at 256 Thurston Road, Rochester or 20 Crossing Creek Drive, Fairport. Job Description
Evaluate and advocate elders’ medical needs and changes in partnership with the medical team and outside services.
Responsible for medication management and distribution
Provide continuous input for individual Elder wellness plans that addresses health and well-being.
Document medical and personal care services provided. Work closely with and communicate service delivery and elder response to the RN and MD.
Ensure medical appointments, specialty consults and discharge summaries are followed-up, communicated and documented in the medical record.
Arrange transportation and schedule medical appointments as needed. Help elders/families access appropriate resources.
Lead team in taking care of Elders, assisting with meals and ADLs.
Assist in planning and implementing daily engagement with community life team.
Provide clear communication with families and providers.
Other duties as assigned.
Qualifications
Current NY State licensure as an LPN in good standing required. Excellent clinical skills including person directed care approach. 2+ years’ experience in a long-term care, assisted living community or ALR homecare setting working with older adults. One year in a management or supervisory capacity.
