The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

Provides professional social work services to patients and their families; collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients; participates in formulating procedures and policies of the Division of Social Service; and participates in educational and research programs, including the orientation and training of new staff and students.Responsibilities Position Title: Social Worker Ambulatory Rheumatology Department: Medicine/Allergy, Immunology & Rheumatology Clinical Unit: Allergy, Immunology & Rheumatology Description of Patient Population: Neonate(s) Adolescent(s) Pediatric(s) X Adult(s) 18-65 X Geriatric(s) 65General Responsibilities The social worker provides clinical social work services to adult patients and their families in the ambulatory service of the Department of Medicine, Division of Allergy, Immunology & Rheumatology, currently located at Lattimore Road, Red Creek, and FF Thompson Professional Office Building Social work services provided include psychosocial assessments, information and referral, psychosocial treatment and other services required to optimize the patients' health care. The social worker collaborates with the Rheumatology health care team in the development and implementation of the care plan of the patients with whom the social worker is active; the social worker will initially focus on with inflammatory arthritises, with a focus on Rheumatoid Arthritis. The social worker also provides consultation to the Rheumatology health care providers regarding patients' care needs and/or community resources. Specific Responsibilities Administrative Essential Functions - Participates in Social Work Division and social work unit staff meetings. - Participates in social work supervision/consultation about his/her work in accordance with plan development with supervisor. - Meets with rheumatology clerical and nursing groups as required. Is actively involved in the AIR Care Coordination Team's meetings. - Participates in required activities of the Social Work Division and Hospital including, but not limited to, timely completion of statistical reports/forms, time sheets and other Hospital or Division forms; mandatory in-service training programs and University Health Service reviews. - Participates in assigned committees of the Social Work Division. Clinical Essential Functions - Performs psychosocial assessments of patients and families referred by clinic providers or identified through high risk screening. These patients include, but are not limited to; elderly or disabled persons in need of home services or protection; persons experiencing emotional crises; and patients with new and/or chronic diagnoses. - Provides social work treatment to patients with problems identified through psychosocial assessments. Treatment may include: crisis intervention; supportive counseling; problem-solving; information and referral, respite or long-term facility based care; assistance with environmental problems. Services are largely provided within the hospital but may include home or agency visits if required by the patients' needs and situations. - Collaborates with other clinic care providers and care coordination team members in development and implementation of interdisciplinary care plans. - Coordinates referrals to and acts as liaison to community agencies regarding services to individual patients. - Provides internal and external systems advocacy as indicated in care plans of individual patients. - Maintains current knowledge of community resources relevant to the clinic patient population. Partakes in community programs and meetings including but not limited to Commongrounds, IBERO, etc. - Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence. - Participates in relevant training and educational opportunities designed to enhance knowledge and skills in protective work. - Participates in the UPP/PEI initiative; collaborates with the hospital high utilizers committee, as well as other Divisions/Departments across the medical center. - Provides social work consultation to other clinic care providers regarding patient problems and - Documents all social work activities in the patients' medical record in accordance with the relevant Social Work Division policy and procedure. Nonessential Functions - In special circumstances, makes visits to patients' homes or community agencies to implement care plans. - Performs other duties as assigned. Teaching Essential Function Provides orientation to AIR nursing and physician staff regarding social work services and selected psychosocial problems and/or resources. Nonessential Function May provide field work instruction to social work students. Professional Accountability, Supervision and Evaluation Supervision/Consultation The social worker's professional performance is supervised by the Manager of Adult Social Work Services. Accountability - The social worker is accountable administratively and clinically to his/her social work supervisor and through him/her to the Director of Social Work and Patient & Family Services for the provision of social work services and adherence to Social Work and Hospital policies procedures and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description. - In addition, the social worker is accountable to the Ambulatory AIR Medical Directors and Nurse Manager for providing social work services in accordance with policies and standards established by the Ambulatory AIR practice. - The social worker is expected to demonstrate the Strong Commitment - "I Care" values. Evaluation Social Work evaluations are conducted by the supervisor at the end of the first six months of employment and at least annually thereafter. Salary and promotional recommendations are made by the supervisor in accordance with guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester. Position Qualifications The social worker must have an MSW from an accredited social work graduate program and must obtain New York State licensure within 18 months of employment. Bilingual (Spanish) preferred.
