Patient Care Coordinator
The Patient Care Coordinator will close gaps in quality measures with insurance companies. They will also submit gap closure data to insurance companies. Other duties may involve looking at and coordinating care through a monthly list from the facility's database of patients' dues for annual wellness visits, annual physical exams and preventative care covered by commercial insurance, Medicare wellness visits, and diabetic follow-up care, increasing the level of patient quality of care.
Job Requirements:
- Ability to communicate in English; both verbally and in writing.
- Basic computer knowledge.
- Ability to type 40 words per minute.
- Knowledge of medical terminology.
- Excellent interpersonal skills.
