Community Care Network is looking for a School Based Case Manager to join our team!

About the Role:
The School Based Case Manager promotes pro-social behaviors and social skills to students within a school-based setting. Provides support to the student to help them achieve academic success. Assist students and families in obtaining recommended service coordination and community support.
Responsibilities:
Provide evaluation, treatment, consultation, community education and training activities in the school and community.
Involves transportation of self and client in the provision of services in a personal vehicle.
Facilitate access and referral to guardians and conservators, self-help groups, advocacy services, and legal advocacy service when available and necessary.
Timely completion of clinical documentation per agency policy to include treatment plan document, clinical contact notes, time sheets, managed care authorizations, discharge summary and all others as needed.
Accept cases as assigned by supervisor.
Attend clinical supervision, and team, department and agency staff meetings as provided.
Qualifications:
Bachelors degree in Social Work, Counseling or related field.
Previous experience assisting with service coordination and supports of students and families.
Proficiency with computers, iPads and other related technologies and experience entering data into electronic medical records (EMR) program.
[]{#_Hlk75512964}Must have a valid drivers license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
*Successful completion of a criminal history background check after hire.
