Within the framework of recovery and resiliency provides client-centered therapy to individuals in the Transitional Aged Youth program.

ESSENTIAL FUNCTIONS:

  • Completes timely biopsychosocial assessments of individuals admitted to the TAY.

  • Collaboratively develops treatment plans and monitors progress of goals.

  • Responsible for coordination of care with internal/external providers and naturals supports.

  • Creates and implements comprehensive discharge/aftercare plan for each individual.

  • Ensures proper releases and service agreements are acquired.

  • Ensure documentation is timely and reflects medical necessity.

  • Attends treatment team and service planning meetings when requested.

  • Performs all other duties as assigned.

  • Demonstrates understanding of Trauma Informed Care in all relationships.

  • Uses the principles of Recovery Oriented Care in work with people served.

  • Ensures high quality care to people in the TAY.

  • Balances the duty to care with the dignity of risk.

  • Demonstrates accountability for personal, professional, ethical and legal standards.

  • Demonstrates support for the philosophy of the organization by adhering to standards, policies, procedures, and established practices.

  • Demonstrates responsibility for own standards of practice by reporting for work on time and avoiding work-related injuries through the application of proper practice techniques and safety policies.

  • Demonstrates respect for client’s rights by maintaining strict confidentiality of client’s information

  • Demonstrates at all times the commitment to providing the least intrusive service intervention that respects the clients’ needs/preferences in the least restrictive setting that supports client and staff safety.

  • Communicates with all colleagues in a respectful, professional, responsive manner that promotes teamwork and supports positive outcomes for clients.

  • Uses a consistent problem-solving focus in working to resolve differences with colleagues and clients.

  • Ascertains that relevant information is reported to program manager and/or the team in a timely manner.

  • Maintains current knowledge in present practice area, i.e., attendance at required hours of organization educational programs and utilizes clinical supervision as a learning experience.

  • Be committed to the mission, vision, and values of the organization.

  • Assures quality in work performed to facilitate the delivery of quality services.

  • Reports accidents and injuries in a timely fashion, demonstrates safe housekeeping practices, notifies supervisor of potential health and safety concerns immediately, participates in health and safety training as required, understands and follows all Sweetser health and safety policies.

  • Ability to support and recognize the important role volunteers play. Sweetser employees work with, support, collaborate with volunteers, and promote opportunities for volunteer engagement.

  • Follow Suicide safer care practices.

EDUCATION:

  • Master’s degree in either social work, psychology, clinical counseling or other comparable mental health program.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

  • LCSW, LCPC or LMFT, Conditional licensure.

  • Maintain a valid state driver’s license.

KNOWLEDGE AND SKILLS:

  • Consistently use the skills of Motivational Interviewing.

  • Demonstrates understanding of Peer Support.

  • Establishes and maintains effective working relationships with colleagues and the community and collaborates effectively within complex systems.

  • Receives and understands professional feedback from supervisors and colleagues and implements supervisory suggestions for performance improvement.

  • Listens and responds to clients, family and colleagues in a sensitive and respectful manner.

  • Recognizes signs of increasing clinical or situational risk and of escalating behavior and utilizes de-escalation and safety implementation techniques effectively.

  • Recognizes variables underlying stressful situations, discerns methods to diffuse or contain them and quickly implements the most effective intervention.

  • Provides crisis/risk assessments and develops/implements stabilization plans that are creative and specific to the persons served and their needs, and promotes health and growth with the goal of reintegration into the community

  • Manages multi-task situations and conflicting demands, copes with stressful situations, prioritizes projects and responsibilities, and meets critical deadlines.

  • Maintains current knowledge in present practice area, i.e., attendance at required hours of organization educational programs and utilizes clinical supervision as a learning experience.

  • Demonstrates ability to work with diverse populations and accepts differences in perspective and values.

  • Communicates ideas and opinions in a clear and constructive manner, both verbally and in writing.

  • Intermediate computer skills required.
