What You Will Do:

  • Functions as a part of a highly qualified team within the Wound Care & Hyperbaric Medicine Program. Provides clinical patient care in accordance with hospital policy for wound healing, utilization review, case management, quality management, and compliance programs to the extent that is medically reasonable and appropriate to do so.

  • Responsible for the operation and implementation of the Wound Care & Hyperbaric Medicine Program – extended care component/satellite clinics. Under the auspices of the Medical Director, the NP provides care for patients in either the outpatient center or in Extended Care Facilities (ECF). Provides evaluation and management services and surgical procedures for wound care patients.

Direct Patient Care Services: Provides medical care to patients, including assessment, comprehensive wound evaluation, interpretation of diagnostic data, and coordination of patient plans of care as well as staff, patient, and caregiver education. Demonstrates knowledge and understanding of Wound Care & Hyperbaric Medicine Program Paths for wound care and the clinical policies and procedures governing wound management. Demonstrates competence in patient/wound assessment, dressing protocols and prescribing of pharmaceutical agents. Utilizes current practice standards and evidence-based practices.

Planning/Organizing/Initiating: Organizes and prioritizes clinical responsibilities to maximize time efficiency and productivity. Implements and evaluates progress toward goals for prescribed plan of care based on the Paths for wound healing. Provides wound care information and collaborates with primary care physicians about the patient’s progress toward wound healing.

Communication: Demonstrates effective verbal and written communication skills. Provides patient information to physicians and caregivers in a timely manner. Written documentation is clear, concise, and completed in a timely manner. Interactions with all health care professional are conducted in a professional manner. Director is kept informed about the program’s progress and concerns/issues which need to be addressed. Provides educational sessions for facility staff relative to wound management and dressing protocols.

General Work Practice: Maintains a safe work environment which is conducive to and complies with all state, federal, and facility policies.

HBOT Duties and Responsibilities: Provides direct medical services consistent with nurse practitioner’s experience and training for the HBOT Services of the Wound Care & Hyperbaric Medicine Program. Services include:

  • Patient evaluation prior to initiation and receipt of HBOT Services that include clinical evaluation, review of medical records, ordering of any laboratory of other necessary tests or procedures, consultation with the patient’s treating physicians, including the patient about the benefits, risks and alternatives to the HBOT Services, and engaging in any other activities preparatory of the administration of HBOT Services;

  • Issues specific orders for HBOT Services and oversees the medical monitoring of the patient’s clinical status during HBOT Services;

  • Manages changes in the patient’s condition, complications or reactions to HBOT Services and coordinates appropriate medical intervention in response to same;

  • Evaluates the results of HBOT Services and coordinates treatment schedule based on the patient’s clinical condition, wound status and other indicators;

  • Documents the medical care and treatment plan as appropriate to the patient’s needs and status as set forth by the Program.

Health and Safety: Adheres to infection control, emergency, safety, fire and disaster plans and policies, completing required initial training and updates per hospital policy. Demonstrates consistent use of Standard Precautions and proper body mechanics and follows OSHA guidelines. Provides a safe environment to protect patients, visitors, and staff.

Nursing Practice Standards: Utilizes nursing practice standards and evidence-based practices, obtains patient information/data, assists in planning, implementation, and evaluation of the patient's care.

ENTEROSTOMAL THERAPIST ESSENTIAL TASKS – perform as needed

  • Provide specialized patient care to ostomy patients and individuals with drains, draining wounds/fistula, pressure ulcers, and skin care for those with incontinence.

  • Collaborate with physicians and other hospital staff with the rehabilitation of patients undergoing urinary or fecal diversions.

  • Pre-operative: teach the patient and family ostomy care, discuss changes in diet and fluid intake.

  • Provide follow-up care in the outpatient setting: counseling, education, stoma/periostomal complications that may require medical intervention.

  • Recommend appropriate interventions to prevent and manage skin breakdown.

  • Responsible for the development, implementation, and evaluation of ostomy/skin integrity-impaired patient care standards.

    What You Will Need:

  • Nurse Practitioner Master’s Level prepared in an area consistent with the population to be serviced. Current Nurse Practitioner license in good standing. Minimum of two years prior experience preferred. Strong leadership, interpersonal, and organizational skills.

  • Must possess a valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.

  • Ability to understand and effectively communicate in both written and oral form.
