Job Summary and Qualifications
GENERAL SUMMARY OF DUTIES: Responsible for providing comprehensive diabetes education (including assessment, treatment, and evaluation) for children, their families, and other health care providers throughout the continuum of care.
DUTIES INCLUDE BUT NOT LIMITED TO:
Assess the educational needs and provide diabetes self-management training to both in-patient and outpatients and their families/caregivers. Identify the patient's status and educational needs using clinical data, history taking and observation.
Communicate diabetes self-care behaviors in relationship to nutrition, exercise, medication use, insulin administration, glucose meter use, glucose monitoring, acute and long-term complications of diabetes and insulin pump therapy.
Participate in the development of education records and teaching materials plus other job duties as assigned.
Participate to the full extent of diabetes education within licensure restrictions, professional scope of practice and the AADE Scope and Standards of Practice and Standards of Professional Performance.
Share and adhere to the mission, values and standards of performance of the practice demonstrated by excellent customer service skills, positive interdisciplinary team interactions and provision of high quality patient interaction.
Maintain professional level of conduct with regarding attitude and communication with staff and patients.
Perform other duties as required. Must read and understand and adhere to all Physician Services policies and procedures.

EDUCATION:
Associates Degree – Nursing
Bachelor’s Degree – Nutrition/Dietetics EXPERIENCE: 3-5 years’ experience in diabetes education - Required
CERTIFICATION/LICENSE:
Registered Nurse (Associates of Nursing) or Registered Dietitian (B.S. Nutrition/Dietetics) – Required
Certified Diabetes Educator – Required
Certified pump trainer with at least one insulin pump manufacturer - Preferred
