About The Role:

The Case Manager is responsible for being sensitive to problems and solving them in a resourceful way, for intelligently planning ahead to prevent problems and increase opportunities, and for going beyond mere compliance and cooperation to active support, encouragement, and teamwork.

Responsibilities:

  • Attend required meetings, conferences, trainings, and other agency functions, as required.

``` - Ensure that all relevant parties are fully informed of important client issues in a timely manner.

``` - Meet with families and children in care to monitor progress and adjustments in placement status.

``` - Assist and support resource families and children in developing relationships which will make possible and promote a safe, welcoming environment.

``` - Maintain close involvement with children, birth families, and resource families in regards to solving interpersonal conflicts and developing and maintaining positive relationships.

``` - Monitor and supervise processes that will ensure goals are met.

``` - Participate in the process of accepting children referred for care in a manner which ensures that the needs, rights, and welfare of children and families are the first priority.

``` - Make appropriate written suggestions for improvement in policies, when appropriate.

``` - Perform additional duties as assigned.

Requirements:

  • Bachelor's Degree (degree in social work or related field preferred)
  • Two years' experience working with children and/or families preferred
  • Eligibility under current PA legal and regulatory requirements
  • Valid PA Driver's License
  • Child Abuse, State Police and FBI Clearances are required for this position and will be paid for by the agency

Physical Requirements:

While performing the duties of this job, the employee is occasionally required to stand; walk; sit. Ability to work in a constant state of alertness and in a safe manner and maintain the standard of care that is appropriate for this position. Must be able to physically respond to residents who are engaging in unsafe behavior. The physical demands described above are representative of those that must be met by an employee to successfully perform the essential functions of this job.
