YMCA Earth Service Corps Case Manager - Caregiver Supports Program in Seattle, Washington

Job Summary

You will provide case management services to families eligible to participate in the YMCA Caregiver Supports Program, including supporting licensed and unlicensed caregivers of youth in foster care. Case management services include coordinating or providing services to enable the child and caregivers to utilize medical, educational, social, clinical, and other services necessary for maintaining the child's physical, psychological, and/or developmental health. You will also provide leadership and support to the greater Case Management team, and ensure that all clients in the Caregiver Supports program are receiving quality case management services.

Hiring Range: $24.00 -- $27.00/hour

Responsibilities

Provide case management and care coordination services, as outlined in the Caregiver Supports Contract, Statements of Work, Handbooks, and Policy/Procedures, to clients enrolled in the Caregiver Supports Program including, licensed and unlicensed caregivers of youth in foster care, therapeutic foster care, intensive in-home stabilization services, and Outpatient Mental Health services.

Maintain a caseload size in accordance with contractual requirements and guidelines.

Ensure clients receive all required screenings, assessments, services, interventions, and opportunities needed to fully meet their physical, psychological, social, emotional, relational, developmental, educational, independent-living, housing, and permanency needs in a timely and effective manner.

In coordination with the Program Director, and in collaboration with the child & family team, ensure clients' transitions are well coordinated and facilitated, including placements & discharges, and ensure all required intake, service planning, and discharge/exit documentation is completed fully and on-time. Provide leadership and guidance in the placement referral process, and ensure that case transfers (of CMs, Intervention Specialists, and Child & Family Therapists) are smooth and as least disruptive as possible to the client.

Ensure clients' parent/child/sibling visitation needs and requirements are met. Ensure natural supports and life-long connections are developed and stabilized.

Coordinate and ensure inclusion of the client's Child/Family Team in the planning and decision-making process. Ensure foster parents/caregivers receive the necessary support services (case-management, care coordination, respite, resources, etc) in order to fully meet the client's needs.

Ensure all services provided are well documented in the client's file in accordance with all WAC and contractual requirements and guidelines.

Ensure all contract required client progress reviews and reports are completed thoroughly and accurately, are well written, and submitted on-time.

Ensure client Referral Agreements and Exceptional Cost Plans are developed to fully represent the client's needs; in accordance with policy/procedure; receive required approvals; and are submitted and renewed on-time.

Ensure each client's file/record is fully in compliance with WAC, DSHS/DLR, DSHS/CA, DSHS/DBHR, KC-BHO, COA Accreditation, and YMCA requirements.

Ensure all clients' information is kept confidential and only released in accordance with HIPAA, DSHS, and YMCA policy and procedure.
