Use your case management and social services skills as a Housing Navigator! The Housing Navigator - Rapid Transitions is a case manager specialist for the Rapid Transitions program at the William Booth Center. The Rapid Transitions program is a housing-focused case management program that directs immediate efforts and resources to address housing attainment, utilizing the minimum assistance needed to address each household's immediate housing crisis. The Housing Navigator works with each individual to identify and refer these households to other resources in the community (mainstream services, benefit services, food assistance programs, childcare resources, etc.) to support on-going housing stability. The Housing Navigator will have a leadership role within the case management team and will have the experience and knowledge to be able to provide support and expertise to other case managers while leading the case management assistants. This position is full-time, non-exempt, and located in Seattle, Washington. Pay Rate: $28.45/hour Duties & Responsibilities Provides intensive housing assistance including all or part of a comprehensive assessment, case management, service authorization, information, and referral necessary for adults to develop and pursue an effective and mutually agreed upon housing plan Provide personal direct service in unusually complex situations or as required by unique circumstances while providing supportive care to residents experiencing personal crisis Provide expert case management for clients with varying degrees of vocational, social, cultural, and/or medical impairments hindering self -sufficiency Post housing case management may include referrals and linkages that assess and determine employability, job readiness, and vocational education and training needs and client economic and social self-support Provide formal and informal referral service to community resources, case management services in a multidisciplinary approach to service delivery Coordinates services with other agencies such as mental health, Employment Security, substance abuse programs, vocational rehabilitation, and developmental disabilities. Provides formal or informal referrals to community services, networks with adult community agency services Maintains client case records, social services system (Bridges & Others), HMIS, and required forms and reports are maintained in the prescribed format. Performs other administrative and operational functions as requested Under direct supervision of the WBC Director, duties will include responsibility for supervision of the shelter monitor (CMA) staff and operational administration Under direct supervision of the WBC Director, provides contract oversight and policy guidance for WBC's referral partners Assists with the monitoring, evaluation, and facilitation of changes in operational and administrative programs to ensure compliance with policy, contractual and regulatory requirements Assists as needed in hiring, assuring adequate coverage of permanent, temporary, and on-call shelter staff Provide direct supervision and training for staf
