The UW MEDICAL CENTER - MONTLAKE - SOCIAL WORK DEPARTMENT has an outstanding opportunity for a SOCIAL WORKER – BEHAVIORAL HEALTH .

WORK SCHEDULE • 30 Hours/Week • Day Shift • This position is expected to last six months POSITION HIGHLIGHTS • Work in a Collaborative Care Model to provide individual assessments and brief therapy, coordination of referrals to clinically indicated services • Serve as a liaison with medical providers to ensure increased adherence to both primary care and behavioral health treatment • Be responsible for conducting regular mood screenings and applying evidence-based techniques such as Behavioral Activation, CBT, MI, and PST, and other interventions as clinically indicated

PRIMARY RESPONSIBILITIES: • Provide specialty consultation to other health care staff and health care teams regarding psychosocially focused aspects of client/patient care • Provide psychosocial support to patients and families • Complete clinical assessments as per department standards for area of service • In collaboration with the multidisciplinary team, develop and implement safe and timely treatment plans • Provide care coordination for patients with complicated social and medical histories • Provide case management and counseling services in specialized programs • Make timely referrals to hospital and/community agencies including those required by mandatory reporting laws such as child and vulnerable adult abuse

REQUIREMENTS: A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND one year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

LEGAL REQUIREMENT: Must hold one of the following credentials issued by the state of Washington: • Licensed Independent Clinical Social Worker
