Description
PeaceHealth is seeking a Admissions Coordinator: Care Management for a Full Time, 1.00 FTE, Day position.
The salary range for this job opening at PeaceHealth is $22.70 – $34.04. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
Job Summary
Responsible for identifying potential swing bed candidates, pre-screening referrals and preparing patients for admission to the swing bed program. Responsible for assistance with discharge arrangements for care facility and home discharge.Provides support to the Care Management Team to ensure that the day-to-day operational functions of the department are completed in a timely manner. Assists with discharge arrangements for care facility and home discharge in collaboration with RN Care Management and/or Social Work. Tracks and issues patient Medicare Notices. Responsible for timely and appropriate documentation within the electronic medical records system.
Essential Functions
Responsible for the coordination of admissions, discharges, and transfers; communicates and prioritizes, and provides for patient privacy, exercises discretion, and assures confidentiality at all times.
Assesses patient referrals to determine that patient meets all admission criteria.
Assists with arranging home health skilled services and durable medical equipment. Assists with post-discharge follow up appointments and arranging new PCP for un-doctored patients.
Coordinates the admission process with patients, families, referring facilities, physicians and Care Management caregivers.
Provides documentation of medical necessity and obtains authorization for treatment re: swing/SNF benefit.
Assists with issuing Medicare notices, confirms PCP and demographic information and faxes hospital clinical updates to PCP and/or payors.
Confirms outpatient therapy or post-hospital appointment arrangements.
Documents discharge planning assistance interventions in CareConnect in a timely manner.
Greets visitors and health care personnel, screens visitors and telephone calls to determine urgency of contact and facilitates appropriate routing or referral.
Performs clerical duties which may include front office support, scheduling assistance, management of daily assignments, operation daily hand time keeping.
Gathers/enters data, retrieves/creates reports as delegated, which may include Concur expense reporting.
Performs other duties as assigned.
Qualifications
Education
Preferred: Associate Degree
Preferred: PT Assistant, Medical Assistant or LPN
Experience
Required: Skilled nursing facility or swing bed background and/or previous health insurance experience.
Preferred: Minimum of 3 years administrative experience in a professional medical office environment.
Preferred: Knowledge of insurance industry (third party payers, state and federal insurance payers). Experience with payer portals.
Preferred: Marketing sales experience/background, experience with computers, statistical compilation, and analysis of data bases relative to quality assurance and effectiveness of programs instituted experience

Excellent verbal, written communication, and problem-solving skills.
Strong organizational skills and demonstrated ability to multi-task in a fast-paced environment.
Ability to understand and apply swing/SNF benefits in the Critical Access setting.
Ability to work with office equipment including computers, printers, and phones.
Proficiency in Windows based PC applications including Word, Excel, PowerPoint, and Outlook.
