Case Managers in the CAT program provide assessment and recovery focused services for individuals and families who may have mental health needs. As part of the team, Crisis Case Managers participate in clinical staffing, treatment recommendations, information and referral inquiries, crisis intervention, and provide consultation within the agency, to community service partners and the public. The primary function of this position is to provide in-depth assessments, short term case management and appropriate referrals to meet identified needs. ***** OTHER EXPERIENCE AND QUALIFICATIONS: This position is a part of the crisis services team of the Behavioral Health Program. ESSENTIAL DUTIES Complete comprehensive initial assessments that reflect the client and families need for mental health services to include presenting problems, psychosocial history, diagnostic impressions, functional screens, and recommendations for treatment. Provide results of the assessment to the client and family as well as treatment alternatives, client\'s rights and responsibilities, the anticipated fees for treatment, and the clinic\'s grievance procedure. Develop a treatment or referral plan in collaboration with the family based on the initial comprehensive assessment to include strengths, treatment goals, measurable outcomes, the frequency and type of service recommended to reach goals.
