Your Role as a Behavioral Health Clinical Team Manager:
As a key leader, you’ll ensure seamless, high-quality home health services while building and inspiring a team of compassionate healthcare providers. You will:
Oversee patient care services, including staffing, care coordination, and referrals.
Ensure patient care starts promptly, following regulatory guidelines.
Supervise and support your team to meet patients’ evolving needs through personalized care plans.
Coordinate and communicate effectively with patients, families, physicians, and other care providers.
Monitor clinical outcomes, update care plans, and ensure compliance with physician orders.
Drive quality improvement and ensure your team meets or exceeds clinical standards.
What You Bring to Elara Caring:
We’re looking for leaders who are passionate about making a difference. To succeed in this role, you’ll need:
Associate’s Degree in Nursing (BSN preferred).
2+ years of home health or behavioral health experience as a Registered Nurse.
1+ year of supervisory experience in a healthcare setting.
Active state Registered Nurse license .
Proficiency in enterprise systems like Workday and HomeCare HomeBase, and familiarity with MS Office Suite (Word, Excel, Outlook).

You will report to the Director Nursing or Clinical Administrator. This is not a full list of responsibilities. Salary for the Behavioral Health Clinical Team Manager ranges from $90,000 to $100,000 , based on experience, with potential for additional earnings. Elara Caring is an equal opportunity employer.
