Intermountain Health Registered Nurse Case Manager Homecare in South Jordan, Utah

Job Description:

The Homecare RN Case Manager serves as a key member of the interdisciplinary team and actively manages and directs resource utilization to achieve the highest quality outcomes during a patient care experience. The incumbent oversees the effective coordination of services and manages issues in the following main areas: admission and discharge, team conference and interdisciplinary plan of care communication, patient and family education/relations, payor relations, and coordination of care for all disciplines and total fiscal management. The incumbent performs ongoing utilization and quality reviews and acts as a liaison between the payor and patient while assuring that cost-effective treatment is provided by the team. The incumbent performs RN Shared Leadership duties by supporting the fundamentals of safety, quality, experience, and stewardship by providing clinical expertise and leadership to their department. The Homecare RN Case Manager has significant leadership responsibilities (e.g., leading change, scheduling, maintenance of staff competencies, new employee selection/orientation, constructive discipline, shared leadership council representation, caregiver check-ins). The incumbent has accountability for coordination of a cohort of caregivers and actively participates in the overall operations of the department, development of department practice, and provision of patient care with specified accountabilities for implementation and outcomes.

Posting Specifics:

Shift Details : Monday through Friday 8AM-5PM Occasional call, weekends and holiday rotation

Minimum Qualifications

  • Current RN license in state of practice.

  • Current Driver's License in the state of practice.

  • Current Hospice Certification or Oasis Certification designation must be achieved within 12 months of hire/transition into this position.

  • Basic Life Support (BLS)

  • RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion.

  • Two years of clinical nursing experience.

  • One year of experience working in Home Health or Hospice setting.

  • Basic computer skills and knowledge of word processing software.

  • Excellent written and verbal communication skills.

  • Independent worker, who is self-motivated, has a positive attitude, and can be flexible in a rapidly changing healthcare environment.

  • Record as a safe driver. Must meet Intermountain Driving requirements.

  • Auto liability insurance with at least the state's required minimum limits.

Preferred Qualifications

  • Bachelor's degree in Nursing (BSN) from an accredited institution. Degree will be verified.

  • Three years of experience working as a case/care manager in a home health or hospice setting.

  • Experience working with third party payers.

  • Knowledge of home- based health services and resources.

  • Auto liability coverage higher than the State's required minimum (at least $100,000/$300,000 is encouraged).

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, and identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

  • and -

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, and driving a vehicle.

  • and -

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.

  • and -

Frequently walks and assists with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling portable equipment, including heavy items. Often required to navigate crowded and busy rooms (e.g., full of furniture, equipment, power cords on the floor). Need to ascend and descend stairs or uneven surface to access patients.

Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
