*LCSW (Clinical Care Coordinator) –Norwalk, Weston, New Canaan PC locations, Fairfield in Connecticut *
Nuvance Health is recruiting part-time Clinical License Social Worker (LCSW) at Norwalk and Weston Primary Care locations in Fairfield in Connecticut. The Clinical Care Coordinator will provide support to the Behavioral and Mental Healthcare department following a Monday-Friday office schedule of 8:30am-5pm in an outpatient setting.
Furthermore, Western CT Medical Group Primary Care Offices serve Newtown, Brookfield, Ridgefield, Southbury and Danbury. The Clinical Care Coordinator will work five hours per week at each location, performing behavioral health assessments, referrals and recommendations for treatment and community services.
Summary: Performs assessments and formulates treatment plan and or discharge/transition plans in collaboration with patient, provider, and possible family and multidisciplinary healthcare team. Provides ongoing monitoring and evaluation of plans to meet the individual needs of patients throughout the care continuum. Assists in resource management through facilitation of care and participation in program planning, data collection and analysis. Provides education to patients/families regarding coverage, care options and community resources.

Qualifications:
Master's degree in Social Work from an accredited school
Minimum of 3 years’ experience
Current CT Licensed Clinical Social Worker (LCSW) in NY & CT

The candidate must have: Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions; Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions; Comfort with the pace of primary care and working with an interdisciplinary team; Excellent communication skills; Knowledge of psycho-pharmacology; Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.
Broad based clinical background and hospital case-management or Crisis Intervention experience, behavioral health community experience, medical social work experience, outpatient treatment, SUD experience, overall experience across the continuum of behavioral health care settings. Excellent oral and written communication skills. Basic computer skills. Prioritize the daily workload and utilize tools dedicated to the position/function. Deal with a high degree of public and patient contact and interaction, and make independent decisions, perform concurrent tasks, manage stressful situations and balance multiple priorities.
