Description:
Position Purpose
The Social Worker at St. Joseph of the Pines plays a key role in coordinating patient care across levels, ensuring quality outcomes, and facilitating effective transitions. Working collaboratively with the patient, physician, and multidisciplinary team, this position focuses on timely and seamless progression through care levels, optimizing treatment quality while managing reimbursement for non-traditional Medicare patients.
What You Will Do
Conduct in-person assessments of patients at admission and ongoing, documenting findings in medical records.
Work with insurance payers to secure necessary pre-certifications and re-authorizations, facilitating appropriate transitions and length of stay.
Act as the main liaison with payers, insurance companies, and case managers, ensuring clear communication with the care team.
Identify and address psychosocial, educational, and discharge planning needs in collaboration with residents and families.
Actively participate in care plan meetings to design individualized care strategies and alternative discharge plans, starting at admission.
Minimum Qualifications
Bachelor's or Master’s in Social Work, or related field (sociology, psychology, rehabilitation counseling).
Three years of clinical social work experience in acute care, plus a year in post-acute care or case management preferred.
Strong interpersonal and communication skills, with expertise in Microsoft Office applications.
