Area Agency On Aging 1-b RN SUPPORTS COORDINATOR in Southfield, Michigan

The RN Supports Coordinator uses person-centered principles to assess the medical, functional, psychological, financial, and environmental needs of participants. Works with participants (or caregiver) to develop a long-term care (LTC) plan and assists the participants to manage their care needs.

Duties Include:

  • The RNSC will assess the functional, psychological, financial, and environmental needs of potential Care Management participants and participants in the Veteran Directed (VA) program.

  • Work with the participant (and his/her selected designee) to develop a person-centered plan; assist and support the participant to manage their care needs. Provide person-centered care options and strategies including the eligibility, frequency, duration, and financing of services. Educate participants/caregivers regarding the participant's health status, health promotion, and risks and benefits of treatment options.

  • Monitor and reassess participants and quality assurance activities. Maintain accurate records in the designated tracking systems, including meeting contractual quality standards. Provide member feedback and concerns regarding service delivery to managers, Care Coordinators and Program Coordinators as appropriate.

  • Adhere to agency, department and program standards. Develop and maintain collaborative relationships with colleagues, team members, ICO Care Coordinators and Manager. Attend monthly meetings and trainings as required.

  • Community based staff performing in-home visits, must be able to accommodate daytime, evening and weekend hours as required. Models AAA1-B values and mission.

Qualifications

QUALIFICATIONS:

  • RN Candidates: Minimum Assoc. or Nursing Degree with current State of MI nursing license required.

  • Candidates with Experience: Must have 2-4+ years of experience in clinical assessments or psycho-social assessments including: home care experience and working with a similar target population.

  • Demonstrates problem solving ability and knowledge of LTC programs, eligibility requirements.

  • Excellent organizational skills and interpersonal skills.

  • Ability to work in a community-based setting, independently with little direct supervision of daily duties.

  • Strong computer skills required in MS Office Word, Excel, and Outlook; will test.

  • Valid drivers license required; will travel within a geographic region utilizing own reliable transportation.
