Key Responsibilities:

  • Holistic Patient Assessment: Evaluate patients to identify psychosocial, financial, and environmental needs, ensuring comprehensive care that addresses the whole person.

  • Individualized Care Planning: Develop personalized care plans that support patients and families through every stage of the hospice journey, revisiting and adjusting as needs evolve.

  • Emotional Support & Counseling: Offer compassionate counseling to patients and families, providing a listening ear and expert guidance through challenging decisions and end-of-life planning.

  • Collaboration & Advocacy: Serve as an advocate for your patients, ensuring they receive the resources, support, and care they deserve. Communicate effectively with doctors, nurses, and other team members to deliver seamless, integrated care.

  • Safety & Reporting: Monitor patient environments, report any safety concerns, and take immediate action to implement safety plans as needed.

Your Qualifications:

  • Education: Master’s Degree or Doctoral Degree in Social Work from an accredited program by the Council of Social Work Education.

  • Experience: At least one year of social work experience in a healthcare setting is preferred.

  • Licensing: Current State License as a Licensed Social Worker.

  • Travel: Ability to travel to patients’ homes, with a valid driver's license and car insurance.

  • Skills: Excellent verbal and written communication skills, with a deep understanding of end-of-life care and DNR protocols (preferred).
