Job Description:

Schedule: Monday - Friday | 8a-5p CST

As a Hospice Medical Social Worker, Your New Career Will Include:

  • Aligned Vision: Ensure all activities align with the vision of Elara Caring and contribute to the success of the Home Health team.

  • Holistic Patient Assessment: Identify psychosocial, financial, and environmental needs through comprehensive patient assessments.

  • Effective Communication: Share significant findings, problems, and changes in patient condition with the appropriate team members.

  • Safety Advocacy: Report unsafe conditions and implement safety plans using available resources for patient well-being.

  • Plan of Care Collaboration: Participate in developing and implementing the Plan of Care to ensure quality and continuity of care.

  • Skilled Interventions: Deliver skilled services and interventions according to accepted standards and physician's orders.

  • Patient and Family Support: Counsel, instruct, and involve patients and families in following the Plan of Care and meeting their social work-related needs.

What is Required?

  • Master’s degree or Doctoral degree in social work from a social work from a school of social work accredited by the Council of Social Work Education

  • Current state license as a Social Worker

  • 1 year of social work experience in a healthcare setting

  • Social work skills as defined as generally social work standards

  • 50% travel required

  • Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
