Hiring Bonus $1,000 (paid after 3 month work anniversary) Position Summary The purpose of this position is to provide mental health community based ARMHS services to assist people living with mental health diagnoses to live as independently as possible. Responsibilities Assess functioning levels, strengths, needs and vulnerabilities of people served to create a functional assessment plan using the individual's participation Develop and implement ARMHS Treatment Plan reflecting needs identified in functional assessment Provide access to referrals, outreach, coordinate and authorize funding for services including medical/psychiatric, vocational, social, educational, financial, legal, housing and chemical dependency Advocate for individual to ensure they are receiving the appropriate services for which they are eligible, in need of and requesting, particularly when communicating with other service providers, family members and other people Complete and maintain accurate case records, reports and paperwork in a timely manner Schedule and complete contracts for service as per the ICSP Complete CEU and training requirements as prescribed by Establish professional rapport and communication with people served, agency Requirements Bachelor degree in human services related field OR at least two years providing services for people living with serious and persistent mental illness At least two years experience CSP or community case management or inpatient experience with a focus on discharge and community resources Meets Mental Health Practitioner qualifications for ARMHS Clear understanding of symptoms and psychosocial rehabilitation principles in providing services for people with major mental illnesses Experience with transitional housing and implementation of federal and state housing subsidies Knowledge of chemical dependency issues, co-occurring mental illness and chemical health and problems associated with survival of domestic and sexual abuse. Hardworking, flexible and ability to work collaboratively with a team Strong written and verbal communication skills Ability to lift and carry at least 25 pounds Applicants must comply with and pass Department of Human Services background check (Rule 11) Applicants must have a valid driver license Must be able to pass a Motor Vehicle Record (MVR) background check Benefits Wage Range: $18.21 - $23.65 per hour - depends on qualifications Medical insurance - single employee coverage less than $90/month Dental insurance less than $17/month 401(k) with a 100% vested employer match Vision insurance Life insurance Flexible Spending Account (FSA) Paid Time Off (PTO) starts at 16 days per year, plus paid and floating holidays Additional Benefits available including disability insurance and more!
