About the Promotores Project Promotores project is designed to deliver culturally competent practices and interventions to improve health and social outcomes for Hispanic mothers and their children across Todd County. This project serves as a bridge between the community and the health care, government and social service systems Mission for Position The Promotores Project Coordinator is a frontline public health worker who has a close understanding of the communities they serve, enabling them to serve as a link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. This person will build a trusting relationship with individuals and families in our community and will be a key member of our patient-centered health teams. Duties & Responsibilities Provide culturally competent practices and interventions to improve health and social outcomes for Hispanic mothers and their children within Todd County: Facilitates screening, intake and documentation processes participants Provides personalized communication and health / social care planning Discuss community resources and referrals available (e.g., Extension classes, food shelf, etc.) and navigate participants to community resources Develop and facilitate screening tools and referral pathways for employment, immigration, health coverage, food, transportation, education and housing security. Improve maternal / child health by monitoring Hispanic patients and proactively working with primary care teams to enhance access to care (preventive care visits, prenatal visits, immunizations, chronic disease management etc.) Deliver Spanish-language interpretation during medical and social care visits Provide Spanish translated community resources to link social, mental, health, job, and education pathways. Participate in trainings that extend culturally tailored approaches to mental health, positive parenting, and evidence-based models that enhance families' abilities to thrive. Grant oversite, reporting and compliance: Track all grant activities and oversee / submit quarterly project narrative and financial reports Document, track and report all grant measurement and evaluation data collection and reporting requirements. Oversee all vendor invoice submissions and financial tracking forms Position Type This is a grant-funded project, with a 32-hours per week position funded through 5/31/2025 to serve the communities in Todd County. Qualifications Qualifications Bi-lingual Spanish / English language required. Certified medical interpreter preferred Experience with community health, public health or related field. Must have strong written and oral communication skills, including preparation and report presentation. Knowledge of personal computers, PC applications, MS Word, Excel, PowerPoint and Project. Must be self-motivated, strong problem solver, and detail-oriented.
