Cleveland Clinic RN Case Manager in Stuart, Florida

As a Registered Nurse (RN) Case Manager, you will work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination. Essential to this role are the core values of patient advocacy, holistic assessment, care planning and the empowerment of patients and families to self-manage. Case Managers also ensure compliance with the standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation.
The hours for this part time position is 8am-4:30pm. The individual hired for this role may work out of North, Tradition or South Hospitals.
Required qualifications for the ideal candidate include:
Graduate from an accredited school of professional nursing..
Current state licensure as Registered Nurse (RN).
Proof of Basic Life Support (BLS) through American Heart Association (AHA) must be presented upon hire.
Certification by an approved entity preferred within one year of eligibility, e.g. CCTM, ACM, CCM. American Heart Association.
3 years or more of clinical acute care experience.
Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Ability to work in a stressful environment and take appropriate action.
Preferred qualifications for the ideal candidate also include:
2 years or more of recent experience in Utilization Review, Quality, or Care Management
