SUNY Upstate Medical University Patient Centered Medical Home Program Manager in Syracuse, New York

Job Summary:

The Patient Centered Medical Home (PCMH) Program Manager is responsible to assure that Upstate University Hospital meets the standards set forth by NCQA (National Committee for Quality Assurance) for Ambulatory Adult Primary Care Clinics. The PCMH Program Manager is fundamental to the continued implementation and evaluation of PCMH accredited ambulatory clinics. This position is responsible for ensuring that clinics meet NCQA requirements and ensures the program meets its stated objectives for successful annual reaccreditation. This individual will analyze, compare and evaluate various courses of action and have the authority to make independent decisions on matters of significance in collaboration with ambulatory director, medical directors, and nurse managers. The PCMH Program Manager will work collaboratively with designated clinics to establish a mutually supportive relationship to oversee all activities in adult Primary Care Clinics related to PCMH. Additionally, the individual will organize and coordinate Quality Program initiatives to include PCMH Care Managers, Care Coordinators, Social Work, Clinic Managers, Medical Directors, staff and providers to ensure a multidisciplinary approach in maintaining PCMH Accreditation. The Program Manager develops and implements a performance improvement program for Adult PCMH Clinics that includes guideline development, process assessment, process correction and monitoring for improvement in preparation of annual accreditation cycle.

Minimum Qualifications:

NYS Licensed/Registered (or eligible) RN required. Bachelor's degree in Nursing required. Three years of post licensure acute care RN experience required. CPR-BLS certification required by time of appointment. Demonstrated success in quality initiatives required. Excellent written/oral communication, analytical and computer skills required. Must be able to communicate effectively verbally and in writing. Candidates should possess advanced skills in spreadsheets, databases, conducting meetings and interpreting quality standards.

Preferred Qualifications:

Nursing specialty certification preferred. Master's degree preferred. Prior experience in a field related to ambulatory primary care setting and quality program management preferred.
