The Safe Parking Case Manager is a member of the service team for Family Housing Network, a housing program of Catholic Community Services. This position is responsible for providing case management service to those families and individuals admitted to the program, coordinating screening, intake and connecting families to Coordinated Entry. The Case Manager is also responsible for maintaining and reporting data for annual and quarterly reports, and providing crisis response to the families and individuals in the program. This position performs assessments and works with Safe Parking households to develop and implement client-centered, goal-oriented Housing Stability Plans, assessing the needs of homeless households, utilizing resources to meet identified individual needs with the objective of obtaining permanent housing. This position works closely with the Program Supervisor and Generalist team to ensure the integrity of the Safe Parking program toward the goals of assisting participants to obtain permanent housing as quickly as possible. The Safe Parking Case Manager will work an 11 AM to 7 PM schedule.

The Holy Rosary Safe Parking program is a part of the continuum of services provided by Catholic Community Services-Family Housing Network (CCS-FHN) to families, veterans, and individuals experiencing homelessness in Pierce County. This program provides safe overnight parking and supportive services to families and individuals without a home who are living in their vehicles. Each household receives a Coordinated Entry assessment and case management navigation emphasizing the primary goal of securing permanent housing. The Holy Rosary site is staffed 24/7 to maintain security, and parking participants will have access to restrooms, showers and laundry.

Responsibilities

Provide Initial Screening and Intake Assessment with Potential Participants:

  • Perform initial screening in-person or over the phone of potential program participants, using the program’s screening tool.

  • Facilitate the Criminal Background Check of potential participants and assess for program eligibility.

  • Notify clients of eligibility/non-eligibility, based on program criteria.

  • Direct clients to site, complete intake paperwork.

    Provide Care Coordination to Safe Parking Participants:

  • Orient participants to the Safe Parking Site, including Safe Parking Community Guidelines, which they must sign.

  • Connect participants with a Coordinated Entry Diversion Conversation and intake as soon as possible.

  • Work with participants to create a client-centered, goal-oriented Housing Stability Plan focused on the household’s needs to achieve permanent housing as quickly as possible.

  • Coordinate with and make referrals to appropriate community agencies.

  • Coordinate permanent housing plan with internal or external permanent housing case managers as needed.

  • Meet at least weekly with participants to monitor goals and update case plan.

  • Serve as a backup to the Coordinated Entry team for facilitating Diversion conversations.

    Maintain program participant files and data according to Quality Assurance standards.

  • Provide data input for Safe Parking clients in HMIS, including referral information.

  • Maintain accurate and timely case notes, and organized client files.

    Communications and Reporting:

  • Provide funder-required reports regarding program, as requested by Housing Services Director.

  • Participate in weekly case management meetings and provide case summaries and client status information.

  • Share relevant information regarding the program to FHN staff.

  • Participate in community-wide meetings as appropriate.

    Provide training and support to Safe Parking volunteers:

  • Act as a liaison between volunteers and FHN staff and families.

  • Update and provide orientation trainings, coordinate ongoing trainings and training materials for volunteers.

    Crisis Response Responsibilities:

  • Provide crisis intervention and after-hours support with program participants and staff as necessary. This includes, but is not limited to:

  • Relaying information regarding schedule anomalies (late arrivals, no-shows etc.)

  • Conflict mediation between participants or participants and staff.

  • Contact appropriate emergency services if needed (police, medical, Child Protective Services)

  • Debrief serious crises with supervisor, write Incident Reports as warranted.

    OTHER DUTIES:

  • Work with Generalist team to maintain a clean, safe space for clients, volunteers, and staff.

Qualifications

Minimum Qualifications

  • BA in a human or social services field or 3 years relevant experience.

  • Experience providing services to low-income/no-income families

  • Ability and willingness to work in a team environment and promote a positive team spirit.

  • Excellent inter-personnel skills including excellent oral and written communication skills

  • Excellent organizational and time management skills

  • Strong computer skills including Microsoft Word and database experience

  • Ability to work with a diverse spectrum of high-need individuals balancing the needs of volunteers with families

  • Positive attitude and willingness to do anything that needs to be done, results oriented.

  • Ability to accommodate flexible hours.

  • Familiarity/comfort with faith community.

    Preferred Qualifications

  • Master’s Degree

  • Experience in case management

  • Knowledge of Pierce County social services and other community resources

  • Bilingual skills (English-Spanish)

  • Experience working with a volunteer base
