Centene Corporation Supervisor, Community Health Services in Tallahassee, Florida

Position Purpose: Supervise daily activities of community health services team and the delivery of a range of activities for individuals enrolled in the health plan for Medicaid or and Medicare. These activities benefit the organizations in impacting individual health outcomes and provides assistance to our clinical team of nurses and social workers.

  • Responsible for coaching and development through shadow community visits and data analysis.

  • Ensure staff follow all guidelines for coaching and supporting members to understand disease prevention and achieve good health outcomes.

  • Monitor staff for appropriate care coordination and follow up.

  • Oversee quality and completion of member health assessments and non-clinical assessments.

  • Audit the performance of staff members.

  • Oversee, coordinate and implement community events such as baby showers, health fairs, community assessments, and other health education events.

  • Monitor quantity and productivity by tracking, reporting, and managing key performance indicators.

  • Identify performance related issues and improve operational efficiencies.

  • Collaborate with various internal departments such as Quality Improvement, Medicare, Marketing, etc. to support communication and improve processes.

  • Facilitate team meetings and provide training to staff to improve or educate on various technical skills.

  • Facilitate and work with other community health workers internally and externally to share best practices, strengthen education and outreach.

  • Participate in meetings and discussions in regards to member’s access local food resources, child care, transportation and other non-medical priority life needs.

  • Ensure compliance with applicable state laws and company policies.

Education/Experience: High School Diploma or equivalent. 5+ years of community health, social work, social services, community advocacy, community outreach, member services, or education, experience; Bachelor’s degree in related field or equivalent and 1+ years of experience preferred. Understanding of the community in designated region through shared experiences or strong desire to help people in vulnerable communities. Prior lead experience preferred. Bilingual skills preferred.

Licenses/Certifications: Valid driver’s license and proof of insurance. Community Health Worker Training/Certification must be successfully completed within 15 weeks of hire date.

Pay Range: $25.96 - $46.68 per hour

Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law.
