Clinical Social Worker Essential Job Duties:

The Clinical Social Worker is responsible for performing assessments appropriate to functional status, diagnosis, care setting, age, development level, and cultural and religious orientation in a timely manner. The candidate will also identify and prioritize the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care. The Social Worker collaborates with Case Managers on discharge planning.

Qualifications

Knowledge, Skills, and Experience:

• Two (2) years social service experience in clinic settings or an acute care hospital (Required)

• Completion of an accredited Social Work Program (Required)

• Current California Licensure as a Clinical Social Worker (Required)

• Effective written and verbal communication skills in the English language

• Ability to define problems, collect data, establish facts, and draw valid conclusions

• CPI Certification within the first 90 days
