After training is complete and you feel comfortable in the role, this LPN position may float to our other outpatient clinics in the greater Toledo area. Schedule: 3 - 4 days/wk, varied 8 - 12 hour days Compensation increases as you complete different levels of training, and take on more responsibility! Performs and records vital signs and all other parameters of ESKD patients as per DaVita policy and/or state regulations; obtains blood samples and culture specimens and prepares for shipment. 25%: Collects patient data including but not limited to assessing and monitoring status of vascular accesses, reviews lab work and patient assessments; seeks guidance when levels are out of "normal" range for ESKD patients 25%: Knows and complies with DaVita policies and procedures in accordance with the state and federal regulations for patient care and services (Medicare, OSHA, HIPAA, or other governing policies) 15%: Provides chronic dialysis therapy within the scope of practice and according to the plan of care; recommends a dialysis plan for ultra-filtration; initiates dialysis via various accesses, complies with all safety measures, monitors patient vital signs applicable to ESKD patients, administers medications per physician instruction if allowed by state agencies, fluid therapy and blood products or other appropriate treatments as needed; documents activity appropriately 5%: Performs clerical duties as needed and reviews with nurse and/or preceptor which may include: filing, lab requisitions, scheduling appointments, answering phones, ordering supplies, billing, copying, faxing, data entry and maintaining patient charts. Dialysis Nurse (LPN) Requirements Active Licensed Practical Nurse license within state of practice. High School diploma, general education development ("GED") credential, or equivalent More than 12 months of clinical nursing experience including 3 months providing nurse care to dialysis patients Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system Additional state requirements/ regulations where applicable Required Preferred Employment at DaVita is contingent upon successfully passing a drug screen and a background check per Federal CMS regulation of dialysis clinics At DaVita, we strive to be a community first and a company second.
