Bon Secours Mercy Health Family Coach - Family Success Network in Toledo, Ohio

  • Primary Function/General Purpose of Position The Family Success Network Family Coach provides direct services to families including administration of screening tools, home visiting, parent education programs, resource management and referral, and family coaching. The Family Coach provides ongoing supportive consultations and case management services. Essential Job Functions

  • Assesses and administers universal screenings, financial literacy training, and/or concrete supports to a caseload of 15-30 families in a specified geographic area

  • Provides interested families a series of one to two home visits each month and access to the fully array of the six pillars (family coaching, parent education, basic life skills, financial literacy, service referrals, and concrete supports ) of service delivery

  • Serves as a Family Coach to develop a prevention services plan, outlining the families’ goals and will utilize evidence-based programming to enhance the families’ protective factors and increase the family’s stability

  • Provides parenting programming and facilitate access to Like Skills training, as well as Family Coaching, which will incorporate Motivational Interviewing to assist the family in moving towards behavioral change

  • Attends meetings amongst collaborative members

  • Collaborates with evaluation staff to facilitate and support the completion of the updated needs and resource assessment for the project and to adhere to evaluator guidance on evaluation/assessment administration

  • Develops rapport with local social service partners and community resources to build and maintain a professional, responsive relationship with key community partners

  • Ensures data collected and documentation of services is consistent with rules and regulations and requirements of program evaluation

  • Submits regular programmatic and fiscal reports as requested by the project director, as well as to other identified partners requesting data

  • Actively engages with family and child serving systems, including attendance at community meetings, such as Family and Children First CouncilThis document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job. Licensing/Certification A valid driver's license is required for community work and home visits. Education Associates Degree, social work, child development, or related field (required)Bachelor's degree, social work, or related field (preferred) Work Experience 2 years of relevant experience providing services to families or children, including prevention or treatment related to child abuse and neglect (required)
