Guthrie Licensed Practical Nurse - Home Health - Part-time(32hours/week) in Towanda, Pennsylvania

Position Summary:

Provides nursing care to patients under the supervision of a Registered Nurse.

Education, License & Cert:

High School Diploma or equivalent required. Graduate of an approved course in practical nursing. Licensed in the State of practice.

Experience:

One‐year experience in home care or public health nursing is desirable.

Essential Functions:

  1. Acts as an associate nurse to deliver nursing care to patients based upon physician orders.

  2. Demonstrates effective time management, flexibility and priority setting in patient care.

  3. As an associate nurse, receives patient assignments and takes responsibility for ensuring the completion of patient revisits, participates in multidisciplinary case conferences.

  4. Demonstrates competence in performance of nursing skills.

  5. Practices within the scope of current policies and procedures of the nursing department and nursing guidelines.

  6. Regarding patient related documentation, the Home Care LPN is responsible for ensuring that he or she remains up to date in the most recent federal, state and agency requirements regarding content and time of submission.

  7. Collaborates with the RN, DOPS, patient and others in planning patient care.

  8. Demonstrates the ability to communicate observations of patients to fellow healthcare providers.

  9. Demonstrates respect and consideration for all patients as individuals.

  10. Maintains collaborative relationship with all members of the healthcare team.

  1. Travel for this position is always required.
