RN Coach – Full Time

Eddy Visiting Nurse & Rehab Association has an exciting opportunity for a Registered Nurse to work with a team of professionals to provide post-acute services for patients with chronic health conditions!

The applicant who qualifies for this position will have experience working within an interdisciplinary team, be familiar with chronic health conditions, and have excellent communication skills.
