RN Coach – Full Time
Eddy Visiting Nurse & Rehab Association has an exciting opportunity for a Registered Nurse to work with a team of professionals to provide post-acute services for patients with chronic health conditions!
Applicant will work within an interdisciplinary team to ensure the best outcome for patients who have multiple hospitals readmissions due to exacerbation of illness. The applicant who qualifies for this position will have experience working within an interdisciplinary team, be familiar with chronic health conditions, and have excellent communication skills. Training will be provided in both Chronic Disease Self-Management and the Eric Coleman Care Transitions of Care Model.
Our screeners at Samaritan and St. Peters hospitals will identify patients who can benefit from follow-up in the community for 30-days post discharge and an in-home visit by the RN Coach. The RN Coach will reconcile medications, ensure a timely follow-up appointment is scheduled with his/her primary care provider, provide teaching about chronic disease self-management, and connect patient/family with resources to help avoid a readmission.
RN Coaches coordinate with nurses in the primary care providers office and health plan case managers to ensure coordination of care and smooth handoffs at the end of the 30-day intervention. Apply today to learn more!
Pay Range: $34.20 - $48.45
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
