Guthrie Social Worker, BS - Medical Social Services - Per Diem in Troy, Pennsylvania

Position Summary:

Assists in meeting the psycho‐social needs of patients in the Acute, Swing and Emergency Department with optimization of Health Services provided to all TCH patients. Interface with RPH, area hospitals and physicians to promote TCH services. To provide intervention with geriatrics families, adults, children, and infants and maintain concordant competencies. Patient Coordination is the collaborative method of assessing, planning, implementing, coordinating and monitoring the flow of services and care required to meet the patient’s health needs to ensure quality, cost effective care. The patient coordinator is knowledgeable of benefit system coverage and is able to evaluate the cost of services for cost containment while maximizing the clients benefit, which demonstrates strong fiscal management. Incorporating leadership qualities into an autonomous, responsible practice that is accountable for healthcare decisions and the flow of the healthcare delivery that address the ongoing needs of the individual being served by the case coordination process.

Education, License & Cert:

Bachelors of Social Work

Experience:

Previous work experience with multigenerational family groups within a hospital/long term care setting.

Essential Functions:

  1. Conducts assessments of the patients physical, psychological, spiritual, social, environmental, financial, and functional status to identify the patients’ needs within twenty‐four (24) hours of admission or referral.

  2. Acts as the patient advocate.

  3. Participates with other members of the healthcare team in developing, implementing, evaluating, and modifying a plan of care with the patient using a multi‐disciplinary, collaborative approach.

  4. Provides direct Social Work services and counseling to patients and their families. Assisting patients and families in the development/maintenance of positive coping skills.

  5. Complete timely social assessment and admission review.

  6. Good communication skills, able to communicate with physicians and outside agencies.

  7. Intervene with counseling and/or supportive services as appropriate.

  8. Determine the patients post hospital care needs and coordinates the discharge planning process as needed. Makes arrangements for equipment needed after discharge.

  9. Communicates patient condition and needs to third party payers and demonstrates knowledge and cost‐effective utilization of healthcare resources.

  10. Daily review of referrals.
