[COMMUNITY: TITLE: Nursing Assistant, State Tested IMMEDIATE SUPERVISOR: Charge Nurse FLSA: Non-exempt POSITION SUMMARY: The primary purpose of your job is to provide a safe environment, give emotional and social support and attend to the resident's physical needs and comfort. Performs direct care to the residents as assigned, in accordance with the residents care plan, and as directed by your Supervisor. Performs other duties as directed by representatives of the community. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Nursing Policies and Procedures. MINIMUM QUALIFICATIONS: Education and Qualifications 1. Current registry of certification to work as a Nursing Assistant in good standing within the applicable laws of the state in which you work, or otherwise possesses the requirements to work as a Nursing Assistant under the laws of that state. 2. Must be a high school graduate or equivalent. 3. Must be at least 18 years old (if 16 years old, must currently be in high school.) 4. Must be able to communicate in English and have the ability to follow verbal and written instructions. 5. Must possess and demonstrate the ability to carry out both verbal and written directions. 6. Must possess and demonstrate good interpersonal skills and attention to detail. 7. Ability to work with supervisors, co-workers and community staff in the performance of duties. 8. Ability to observe proper safety and sanitary techniques. 9. Ability to work hours as scheduled based on the requirements of the position/assignment. 10. Must not pose a direct threat to the health and safety of others in the workplace.     1. Ability to verbally communicate with residents, families, the public and other members of the organization.

    1. Must possess basic computer skills and ability to navigate through various software programs, including electronic health record software.

{03775084 - 1} Page 2 of 4; Nursing Assistant, State Tested ESSENTIAL FUNCTIONS: 1. Works alone giving total care or assists residents with:

  • a. Bed bath, tub bath or shower.
  • b. Oral care, hair and nail care, shaving, dressing and undressing.
  • c. Incontinence care.
  • d. Ambulating, transferring, positioning, transporting by using assistive devices such as a walker, wheelchair, Hoyer lift, gait belt or lift sheets. Restorative care as directed.
  • e. Meal service, passing trays, feeding residents, recording intake and output, passing water, weighing residents.
  • f. Taking and recording vitals, as directed.
  1. Answers all call lights promptly.
  2. Observes and reports the following immediately to Supervisor: any incidents/accidents, physical and/or behavioral changes in a resident, grievance or complaint, refusal of treatment or allegation of abuse, neglect or misappropriation.
  3. Works tactfully and cooperatively with residents, families, visitors, and the entire staff throughout the organization.
  4. Assists residents in preparation to participate in act vity programming, and participates as needed.
  5. Perform routine housekeeping duties (i.e., bed making, clean bedrails, over bed table, night stand, etc., that relate to nursing care procedures) and relocate resident and belongings, as needed.
  6. Maintains clean and orderly resident rooms daily and assures that the call light cord is within easy reach of the resident at all times.
  7. Review resident care plans frequently to determine any changes in the residents daily care routine.
  8. Treat all residents fairly, and with kindness, dignity and respect and in compliance with resident rights.
  9. Maintain resident health information as required by law.
  10. Complete and/or attend all training and education as assigned and as otherwise required by applicable law, rule, or regulations.
  11. Adhere to the community's Code of Conduct, Business Ethics policy, HIPAA Privacy and Security policies, Safety Policies, Infection Control policies, department and community policies and procedures, including documentation and reporting responsibilities.
  12. Maintain regular attendance, unless excused under the communitys attendance policy, and/or as required under the Family and Medical Leave Act, or as a reasonable accommodation under the Americans with Disabilities Act, or as excused under other applicable law, with advance notice of absence as required by community policy to the extent permitted by applicable law.

ENVIRONMENTAL FACTORS: 1. Requires working under stressful conditions and, at times, irregular hours or in other positions temporarily, when necessary. 2. Is subject to frequent interruptions. 3. Is involved with residents, personnel, visitors, visiting government agencies, under all conditions and circumstances. This includes working with ill, disabled, elderly, emotionally upset and, at times, hostile people within the community. 4. Is subject to call back during emergency circumstances (i.e. severe weather, evacuation, postdisaster). 5. May be exposed to infectious waste, communicable diseases and/or body fluids per the comm
