GENERAL STATEMENT OF DUTIES: This position is primarily located at Trumbull County Juvenile Justice Center and is funded by the Trumbull County Mental Health and Recovery Board. Responsibilities include providing recovery services, facilitating crisis intervention, providing community support, discharge planning and necessary evaluations of psychiatric, medical, vocational and health needs.
[ESSENTIAL RESPONSIBILITIES:]{.underline}
Monitor and coordinate the delivery of necessary evaluations (psychiatric, medical, vocational, health, etc.) and assessments in a timely manner to ensure that client's needs are adequately identified while housed in the detention facility.
To maintain current working knowledge of all F&CS policies, TCJJS policies and COA standards, OMHAS standards and relevant laws.
Maintain records related to cost, services providing and supporting documentation to meet both F&CS and TCJJC requirements.
Conduct individual and group case management services in accordance with the needs of TCJJS.
Provide supportive interventions during periods of crisis to assist the client in stabilization.
Assist the individual in achieving objectives and maximizing age-appropriate independence and productivity through support and training in use of personal resources.
Make referrals to other services within the community and within the agency as indicated.
Attend and participate in staff meetings, in-service training, and supervision.
Perform other assigned and related duties specific and customary to the department in which they are assigned.
Requirements
QUALIFICATIONS:
Ability to establish and maintain effective relationships with clients and significant others.
The ability to function as part of an organization by working with others & adhering to established policies and procedures.
Valid driver's license, reliable transportation, and adequate levels of insurance and insurability.
Must be able to meet agency requirements for criminal back ground check (Ohio Bureau of Criminal Identification & Investigation).
MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: This position requires a* Bachelors degree* in a mental health related field. License in social work preferred.
MINIMUM EXPERIENCE REQUIREMENTS: Prior community support experience preferred.
PHYSICAL REQUIREMENTS: Ability to communicate: read, write, and speak clearly. Ability to type and use computer. Ability to walk up and down stairs and sit for extended periods of time. Must be able to bend and lift up to 25 pounds with or without assistance. Ability to drive.
