High Value Network Associate Director (local remote - Philadelphia and surrounding counties)

Join our team as a High-Value Network Associate Director! In this role, you will play a crucial role in maximizing the quality-of-care performance and collaboration among healthcare agencies and facilities in the Tandigm population service area. Your primary focus will be on high-impact Hospitals, Skilled Nursing facilities, and Home Healthcare agencies.

As the High-Value Network Associate Director, you will be responsible for ensuring performance outcomes, managing relationships, and providing ongoing support to organizations in the continuum of care. Your goal will be to foster the right care at the right place with optimal transitions for the Tandigm population.

You will guide facility and agency leadership in their transition from fee-for-service to value-based care as a population health subject matter expert. This will involve consulting on process changes that positively disrupt the status quo, leading to improved patient experience, reduced unnecessary medical spend, and better outcomes, such as lower readmission rates. Additionally, you will organize and facilitate Joint Operating Committees and work with analytics to bring actionable data to stakeholders, supporting process improvement initiatives linked to improved patient healthcare outcomes.

The successful candidate will demonstrate a strong sense of collaboration with Tandigm leaders, teammates, and all stakeholders. The ability to foster positive relationships with a high level of emotional intelligence is essential. Additionally, demonstrated abilities in project management and change management are required.

The AD will need to accommodate client availability for regular meetings with a flexible work schedule Monday through Friday and can expect to spend an average of 20-25 hours "on the road" each week visiting practices.  Every candidate must have a valid US Driver's License and personal vehicle for their own use.

* Essential Functions*

  • Consistently exhibits behavior and communication skills that demonstrate Tandigm Health's commitment to superior customer service, including quality, care and concern with each and every internal and external customer.
  • Serves as a trusted and strategic partner for facility and agency leadership within the Tandigm service area.   
  • Understands the clinical and financial aspects of Population Health and be comfortable working with all levels of management and staff.
  • Effectively collects and analyzes facility and agency performance management metrics within each segment and works with Tandigm Clinical Leaders to develop strategies.
  • Understands facility and agency's unique operational challenges to provide them with timely and proactive advice, direction, and action planning for operational improvement.
  • Facilitates workshops with stakeholders to develop solutions for operational improvement.
  • Effectively leads Joint Operating Committees with both Tandigm and facility and agency leadership.
  • Maintains an expert-level understanding of Tandigm's care programs and clinical products to make connections that improve outcomes. 
  • Works cohesively with Tandigm Clinical and ACO leaders to educate stakeholders in the use of the Direct to SNF and Medicare Shared Savings 3-Day Waiver.
  • Performs onboarding of new facilities and agencies to Joint Operating Committees and processes as assigned. 
  • Provides Tandigm leadership with proactive and t mely analysis and updates for all high-volume entities, coupled with strategies to assist each in achieving predetermined goals.
  • Supplies Tandigm staff with expert field knowledge that contributes to the ongoing development of clinical strategies.
  • Uses, protects, and discloses Tandigm's patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards and protects Tandigm assets from unauthorized access, disclosure, modification, destruction or interference.
  • Adheres to and responsible to abide by all information security policies, processes, and/or activities.
  • Reports security events or potential events or other security risks to the organization to management.
  • Performs additional duties as assigned.

Required Qualifications:

Minimum

  • Bachelor's degree from a four-year college and/or a professional certification requiring formal education beyond a two-year college.
  • Minimum five years in a leadership role in a healthcare agency or facility

Preferred

  • Master's Business Administration (MBA), Masters Public Health (MPH), or Masters Health Administration (MHA) preferred.
  • Population health experience

     

Licenses/Certifications

  • Valid U.S. State Driver's License as well as proof of liability and property damage insurance on any vehicle used for business purposes are required.
  • Nursing Home Administration, Nursing, Physical Therapy or Social Work license preferred.
  • Salesforce experience preferred.
