Description
This is a Public Health Solutions funded position as part of the Undetectables Program at GMHC. The goal of this program is to connect clients to comprehensive services which promote U=U. TheOutreach Worker/ Case Managerwill conduct outreach to enroll eligible patients into the Undetectables program. The Outreach Workers/Case Manager will also provide essential services of the program including care/service plans creation, case conferences with medical providers, treatment adherence counseling, modified directly observed therapy (mDOT), and distribution of client incentives.
Essential Job Functions
The following duties are mandatory requirements of the job:
Conduct in-reach within GMHC, and outreach at local health fairs, joint outreaches at our community partners’ sites and through our targeted mobile testing van outreaches
Recruit, screen, and enroll eligible clients into the Undetectables program
Complete care/service plans in collaboration with client, with the goal of increasing client’s self-efficacy and ability to stay and remain undetectable
Perform case conferences with medical providers
Provide treatment adherence counseling sessions to clients in both individual and group settings
Conduct modified directly observed therapy (mDOT) sessions with clients who need additional support with treatment adherence
Distribute client incentives to eligible undetectable clients
Provide referrals for appropriate medical, preventive and psycho-social services, both internal and external
Maintain documentation in EMR of all client services in accordance with agency and funder requirements
Assist in establishing and maintaining collaborative partnerships with community service providers
Assist with special projects and perform other related duties as assigned by supervisor
Requirements
Education and Certification
An Associate/Bachelor’s Degree in a relevant field is preferred; OR
A minimum of four years’ experience working in a similar social service setting/role outreaching hard-to-reach populations.
Special Skills and Knowledge
In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:
Exceptional verbal, written, communication and interpersonal skills
Knowledge of computer software (Microsoft Office Suite including Word, Outlook, Excel)
Knowledge of and interest in HIV/AIDS, sexual health education, PrEP/PEP
Ability to organize and manage personal workflow and necessary travel
Work as part of a team
Bilingual English/Spanish preferred

This is a Hybrid position which requires some days in office
