Visiting Nurse and Hospice for VT & NHcovers more than 70 towns delivering superior nursing, rehabilitation, hospice, and personal care services with proven effectiveness, integrity, and compassion. Physical Therapistat VNH, under the direction of the Clinical Manager, screen, evaluate, re-evaluate, and treat patients referred by a licensed physician. They coordinate and communicate treatment activities with health care team members and the patient as well as work closely with the interdisciplinary team to ensure high quality patient care.

The PT:

  • Evaluates the patient's need for physical therapy and develops appropriate treatment plan.

  • Carries out appropriate treatment program in accordance with agency policies and professional standards of care.

  • Instructs family members, PTA's, Home Health Aides, and other caregivers in appropriate physical therapy techniques.

  • Coordinates physical therapy progress with other caregivers in appropriate physical therapy techniques.

  • Refers patient to other professionals or agencies when appropriate.

  • Monitors and periodically re-evaluates patient's physical and emotional progress.

  • Prepares reports and maintains records in accordance with agency policy.

  • Communicates with the physician and other health care providers on a regular basis regarding the patient's progress.

  • Provides consultation and in-service education to staff on physical therapy procedures. Contributes to the professional development of new staff and health professional students.

  • Provides consultation and direct service to other individuals and groups in community, clinic, school, industrial and other settings in conjunction with the agency's program.

  • Supervises Physical Therapy Assistant.

Minimum Qualifications:

  • B.S in Physical Therapy

  • Must be professionally licensed in both VT and NH

REQUIRED LICENSE/CERTIFICATIONS/REGISTRATIONS SKILLS (Tracked at the department level)

  • PT License in both VT and NH
